Using blood pressure management as an example, a community pharmacist, GP, and primary care team had a practical and interactive conversation about how system working enhanced Access and Recovery services and improved patient care in NHS Black Country ICB. Why it’s important
Integrating the NHS Community Pharmacy Hypertension Case-Finding Service with community pharmacy prescribing improves access, reduces pressure on GPs, and supports earlier intervention to prevent cardiovascular disease. It also empowers community pharmacy to deliver more clinical services, aligning with the NHS 10 Year Plan.
