Collaborative Audit of Omitted and Delayed anti-microbial doses, in acute, community and mental health settings.
This large point-prevalence collaborative audit on omitted and delayed doses was carried out in December 2010. 54 acute, mental and community health trusts across 4 SHAs reviewed 8748 patients prescribed 21,825 doses of antimicrobials to measure rates and reasons for omissions of all doses and first doses as well as delays in a 24 hour period. Data was analysed by care sector and care areas and identified common themes. Recommendations were made for trusts to review processes around documentation and supply and the importance of addressing this challenging issue on a multi-disciplinary multi-pronged approach.
