Learning Outcomes

At the end of this month’s webinar participants should be able to

  • Explain the ambition to create and operate as a single Pharmacy workforce across an STP/ICS (aligning with IPMO plans) to improve collaborative working and improve medicines outcomes for frail older people
  • Outline a plan using a bottom-up approach to identify, build relationships and share expertise with local frailty specialists in the acute trust or other care settings
  • Identify specific learning or knowledge that will empower them to constructively challenge or query discharge medication (within their scope of practice) that pose a risk to frail older people
  • Apply learning to undertake interventions in their daily practice to resolve common medicines related issues and reduce risks associated with problematic drugs (e.g opiates, antipsychotics) at the interface of care
