Description:                 Caroline Dada is Medication Safety Officer and Lead Pharmacist for Specialist/Community Mental Health Services, Gender Identity and STOMP Leeds & York Partnerships NHS Foundation Trust.  She has successfully introduced the roles of the Pharmacist into Community mental health teams (CMHTs) and medicine optimisation liaison pharmacy roles in primary care. These team support and enhance the seamless transfer or patient from secondary care back to primary care by working closely with organisations in primary care.

In this webinar, Caroline will highlight key issues around transfer of care in mental health that all pharmacy staff should be aware of
