Carbamazepine; Appropriate second generation antipsychotic
Moderate level of published evidence of use in breastfeeding
Long half-life increases risk of accumulation in breastfed infants
Significant amounts in breast milk
Case reports of adverse effects in breastfed infants include: cyanosis, lethargy, and ECG changes; lithium intoxication in dehydrated infant; elevated TSH levels; hypotonia and fine motor delay. In most of these concomitant drugs may have been involved
Avoid use in dehydrated infants
Monitor infant for tremor, involuntary movements, cyanosis, effects on muscle tone; and infant serum lithium, creatinine and TSH levels
