This drop-in session explored the process of investigating deviations, establishing root causes and writing the investigation report.

Appropriate investigation of deviations and establishment of root causes helps inform corrective and preventative actions. The process also helps to:

  • facilitiate continuous improvement and assurance of quality
  • demonstrate compliance with regulatory and governance expectations
  • support safe outcomes for patients
