These videos aim to complement the information in our article on a person-centred approach to polypharmacy and medication review. They demystify person-centred structured medication reviews (SMRs) to tackle inappropriate polypharmacy. Introduced by Sheetal Ladva, Specialist Pharmacist Medicines Optimisation at SPS, our subject matter expert Lelly Oboh, Consultant Pharmacist for the Care of Older People, talks you through the 7 steps in more detail.

Person-centred approach to SMR

In this video we introduce you to a practical, person-centred 7-step approach to undertaking SMRs. We explain what will be covered and why we have created these videos.

Step 1 – assess the patient

Here we guide you through the first step of the SMR framework – assessing the patient. Lelly talks about what information to gather, how to build rapport, and how to understand the patient’s individual needs and priorities.

Step 2 – agree goals

This video focusses on step 2 – goal setting. Lelly guides you to explore what matters most to the individual and how to set realistic, meaningful goals. This will keep the SMR focussed, person-centred and aligned with their priorities.

Step 3 – identify problematic medicines

Here we look at ways to identify potentially inappropriate medicines, using evidence-based tools. This third step ensures treatment stays safe, appropriate and person-centred.

Step 4 – assess individual risks and benefits

This episode covers step 4 – personalising the evidence through shared decision making. Lelly shows you how to tailor clinical guidance to the person’s needs, using a range of tools. This step ensures deprescribing decisions are evidence-based and aligned with what matters most to the patient.

Step 5 – agree actions

This step concentrates on step 5 – agreeing actions. Lelly guides you through ways to safely stop medicines, following shared decisions, using well-known resources. We also discuss tools to support patient understanding and monitoring.

Step 6 – communicate with others

This step focusses on communicating the plan. Lelly shows you how to share information clearly with the wider multidisciplinary team to support safe, co-ordinated care. We also explore tools for effective handover and follow-up.

Step 7 – monitor, review and adjust regularly

The final step covers monitoring outcomes. Lelly explains why follow-up is essential to ensure safe and effective deprescribing, that is aligned with the person’s goals.
